Job Responsibilities 

  • Present to community groups providing information on funding options and services available by Aveo 
  • Increase our value proposition for people coming into Aveo communities and empowering residents by offering choices 
  • Meet with prospective consumers and their representatives to understand and assess their care needs to deliver tailored care plans 
  • Provide case management for consumers who chose Aveo Care at Home to manage their government funded home care package 
  • Assess, review and evaluate care and services provided to consumers 
  • Prepare and maintain records, documents and reports as required 
  • Ensure a consumer-directed approach to service delivery and quality care in accordance with Aged Care Quality Standards, Legislations and Aveo’s care governance framework.

Job Requirement

  • Current AHPRA registration 
  • Demonstrated recent experience as a Registered Nurse in a community nurse or case management role in the community sector is essential 
  • Clinical nursing experience within a community care setting 
  • Home Care Package case management experience in a community setting desirable 
  • Comprehensive knowledge of the Commonwealth Home Care regulatory framework, including funding and Aged Care Quality and Safety Commission requirements 
  • Demonstrated problem-solving skills related to clinical service delivery in the home 
  • High level communication, interpersonal and organisational skills 
  • Ability to liaise proactively with key stakeholders and build relationships to achieve required care outcomes 
  • Demonstrated understanding of continuous quality improvement practices and application 
  • Commitment to customer service and achieving positive consumer outcomes
  • Experience in networking and promotion of services 
  • IT literacy with an ability to manage spreadsheets and use Microsoft Office applications 
  • Current Police Check certificate, or willingness to complete one 
  • Current CPR certificate.
